Pay range : $43-$45/hr

Must reside in Dona Ana County, NM
Assess and develop care plans for members.
Conduct phone, home, or in-person visits.
Coordinate care with healthcare professionals.
Monitor and update care plans regularly.
Support members with behavioral health and wellness programs.
Requirements:
Certifications: LCSW, CCM, LPC, LMFT, CHEP, APSW, or Respiratory
Therapist.
Experience: 1-2 years in case management, disease management,
managed care, or medical/behavioral health.
License: Valid driver's license with reliable transportation.
Education: Bachelor's or Master's in social sciences, psychology,
public health, social work, or related field.
